Seeking Physical Therapists for In-Home Care

PT/ PTA

Offer: Flexible schedule. $50 - $70 per visit. Full time and per diem available.  

Requirements: Must hold a current PT/PTA license through Washington State, must have one year of experience required in your field, six months to one year experience in home health preferred, 

 

Duties: Establish the Care Plan with team and coordinate and implement appropriate alternatives. Observe client for changes in condition and reports such changes to the Physician and/or Nursing Supervisor, obtain orders for changes to the Plan of Treatment, and documents all observations and interventions in the clinical record. Develop and keep current orders for medications and treatments as prescribed by the Plan of Treatment. Implement and revise the client's Plan of Care with Physician approval as appropriate. Use appropriate preventive, therapeutic and rehabilitative nursing procedures and keep client's physician, family and the Clinical Supervisor informed of care being provided. Assist and instruct client and family members in nursing care required to promote client's health and safety. Document nursing observations, interventions and evaluations of the care being provided in accordance with agency policy.
